Sensitive Teeth: What Causes the Sharp Pain, What Works and When to See a Dentist

Cold drink. Sharp pain. Sweet food. Another jolt. Even a breath of cold air can make one tooth feel electric.
Sensitive teeth are common — but “sensitivity” is a symptom, not an explanation. Sometimes exposed dentine is the problem and a good desensitising toothpaste really can help. Sometimes the pain is coming from gum recession, tooth wear, whitening, decay, a crack or another dental problem that needs something different. The useful question is not just “Which toothpaste should I buy?” It is “Why is this tooth sensitive — and what will actually stop it?”
In 30 seconds
- Dentine hypersensitivity usually causes a short, sharp pain when exposed dentine reacts to cold, air, touch, sweet or acidic foods.
- Common contributors include gum recession, tooth wear, acidic foods and drinks, aggressive brushing, gum-disease treatment, orthodontic movement, whitening and grinding.
- Desensitising toothpastes containing ingredients such as stannous fluoride, potassium compounds, arginine or calcium sodium phosphosilicate can reduce sensitivity in many people.
- Use the product consistently. Some treatments take time rather than working after one brushing.
- If one tooth becomes suddenly painful, pain lingers after the trigger has gone, you cannot bite comfortably, or the tooth looks cracked or decayed, see a dentist rather than assuming it is ordinary sensitivity.
- Whitening can temporarily make teeth more sensitive.
- Diet, acid exposure, reflux, gum recession and the wider nutritional picture can matter. HTMA may sometimes add information about broader mineral patterns, but it does not identify a cracked tooth, cavity or exposed dentine.

What is actually happening inside a sensitive tooth?
The visible crown of a healthy tooth is covered by enamel.
The root is normally protected by the gum and a thinner layer called cementum.
Underneath sits dentine.
Dentine contains thousands of microscopic channels called tubules.
When dentine becomes exposed and those tubules remain open, changes in temperature, pressure or chemistry can move fluid inside them.
That movement can stimulate nerves deeper inside the tooth.
The classic result is:
A sudden, sharp pain that disappears when the trigger disappears.
Sensitive teeth are often not “weak nerves”. The problem may be that the protective covering over the dentine has been lost.
What causes sensitive teeth?
There is no single cause.
The important step is working out how the dentine became exposed — or whether the pain is actually coming from something else.
Gum recession
When the gum moves away from the tooth, part of the root may become exposed.
Root surfaces are much more vulnerable to sensitivity than enamel-covered crowns.
Tooth wear and erosion
Repeated acid exposure can gradually wear away protective tooth structure.
Common sources include:
- Frequent fizzy drinks
- Fruit juices and acidic drinks
- Frequent citrus or sour foods
- Sports and energy drinks
- Repeated vomiting
- Acid reflux
Aggressive brushing
Scrubbing hard does not make teeth cleaner.
Repeated trauma, particularly around an already vulnerable gumline, may contribute to recession or tooth wear.
Grinding and clenching
Grinding can contribute to tooth wear, cracks and other damage that may present as sensitivity.
After periodontal treatment
When swollen gums become healthier after periodontal cleaning, previously covered root surfaces can sometimes become more exposed.
Sensitivity may temporarily become more noticeable.
Orthodontic movement
In some people, orthodontic tooth movement can be associated with local gum recession and exposed root surfaces.
Teeth whitening
Sensitivity is a recognised side effect of whitening treatments.
It is often temporary — but if your teeth are already sensitive, it deserves consideration before bleaching.
What does gum recession have to do with sensitive teeth?
A lot.
When the gumline moves backwards, the root surface loses some of its natural protection.
Cold water, air or touch can then reach exposed dentine much more easily.
That is why sensitivity and recession often appear together.
If the sensitivity began because the gumline has moved, treating the pain is useful — but understanding why the gum receded matters too.

Which sensitive-tooth toothpaste ingredients really matter?
This is where toothpaste advertising becomes confusing.
Different products use different active ingredients and mechanisms.
The evidence does not say every “sensitive” toothpaste is identical.
Systematic reviews and network meta-analyses support several active approaches.
Stannous fluoride
Stannous fluoride can help block exposed dentinal tubules while also providing fluoride protection.
Clinical evidence supports its use for dentine hypersensitivity.
Potassium compounds
Potassium salts are intended to reduce the excitability of nerves associated with the exposed dentine.
They are among the longest-established desensitising ingredients.
Arginine
Arginine-containing formulations aim to help seal or occlude open dentinal tubules.
Evidence supports benefit in particular formulations.
Calcium sodium phosphosilicate
Sometimes described as bioactive glass, this material can deposit mineral-like material over exposed dentine and help block the tubules.
Hydroxyapatite
Hydroxyapatite-containing toothpastes are increasingly marketed for sensitivity and remineralisation.
Some clinical studies are encouraging, but formulation and evidence quality vary.
Do not judge a sensitive toothpaste only by the brand name. Look at the active ingredient — and give it enough time to work.
What can you do now?
1. Use a genuine desensitising toothpaste twice daily
Choose one with an evidence-supported desensitising active ingredient.
Use it consistently rather than switching brands every two days.
2. Spit — do not immediately rinse everything away
After brushing with fluoride toothpaste, spit out the excess.
Do not immediately rinse your mouth with lots of water.
That allows fluoride to remain in contact with the teeth for longer.
3. Stop scrubbing
Use a soft or medium toothbrush and gentle technique.
More force does not equal more cleanliness.
4. Look at your acid exposure
Ask how often your teeth meet acid across the day.
Five acidic drinks spread across a day can be more troublesome than one exposure with a meal.
Reduce unnecessary frequency.
5. Do not brush immediately after an acid attack
After vomiting or significant acidic exposure, rinse gently and give the mouth time to recover before brushing.
6. Pause whitening if it is clearly making things worse
If sensitivity begins or increases during whitening, discuss the regimen with the dental professional supervising it.
7. Watch the pattern
Several generally sensitive teeth after whitening is a different pattern from:
- One tooth suddenly becoming painful
- Pain when biting
- Pain waking you at night
- Pain that lingers after cold or heat
Those patterns deserve a closer look.
Common medicines and treatments: benefits, limits and side effects
There is no standard prescription tablet that “cures” ordinary dentine hypersensitivity.
Most effective treatments act locally on the tooth or on the cause of the exposed dentine.
Desensitising toothpaste
Benefit: convenient home treatment with good evidence for several active ingredients.
Limit: it usually needs repeated, consistent use and cannot repair decay, a crack or diseased dental pulp.
Side effects: these products are generally well tolerated, but individual formulations can cause taste changes or local irritation in some people. If a product clearly irritates your mouth, stop using it and ask a dental professional about an alternative.
Fluoride varnish or professional desensitising agents
A dentist may apply fluoride varnish or another professional desensitising material to an exposed sensitive surface.
Benefit: treatment is placed directly onto the problem area and may reduce sensitivity.
Limit: relief may not be permanent, particularly if the cause — such as erosion, recession or aggressive brushing — continues.
Side effects: fluoride varnish is generally well tolerated when applied appropriately. It can leave a temporary coating or taste after application.
Bonding, sealants or a filling
A dentist can sometimes cover an exposed or damaged area with a restorative material.
Benefit: physically shields sensitive dentine and can also repair root wear or decay.
Limit: restorative materials can wear, stain, chip or eventually need replacement.
Gum grafting
If sensitivity is associated with significant gum recession, periodontal root-coverage treatment may be an option in selected cases.
Benefit: can provide additional tissue over an exposed root and may reduce sensitivity as well as improve tissue stability.
Limit: this is a surgical procedure and complete root coverage is not guaranteed.
Side effects: temporary soreness, swelling and healing discomfort are expected after oral surgery.
Root-canal treatment
Root-canal treatment is not a routine treatment for simple dentine hypersensitivity.
If the dental pulp is diseased or irreversibly inflamed, however, the pain may have been something more than ordinary sensitivity from the start.
The best treatment depends on the cause. A toothpaste can help exposed dentine. It cannot fix a crack, cavity or diseased pulp.
Can whitening make sensitive teeth worse?
Yes.
Tooth sensitivity is a recognised side effect of professional and home whitening.
It is often temporary.
But if your teeth are already sensitive, tell the dentist before whitening begins.
Recent evidence suggests that whitening protocols can sometimes be adjusted to reduce sensitivity — for example by changing peroxide concentration or exposure, and by using suitable desensitising approaches.
Do not keep pushing through significant whitening pain simply because you want whiter teeth. The whitening plan can often be adjusted.
What about diet, reflux and mineral balance?
This is where the wider wellness picture becomes useful.
Sensitivity can be partly driven by loss of protection over the dentine.
Diet can influence that process.
Acid frequency matters
Repeated exposure to acidic foods and drinks can soften and erode tooth surfaces.
Common examples include:
- Fizzy drinks
- Energy and sports drinks
- Fruit juices
- Frequent citrus
- Vinegar-heavy drinks or tonics
- Frequent sour sweets
Reflux and vomiting matter too
Acid does not only come from food.
Gastro-oesophageal reflux and repeated vomiting can expose teeth to stomach acid.
If this is happening regularly, the answer is not simply a stronger toothpaste.
Mineral nutrition supports the wider tooth environment
Calcium, phosphate, vitamin D, magnesium and other nutrients participate in normal mineral and bone biology.
Saliva also helps buffer acids and supports remineralisation.
But a sensitive tooth should not automatically be interpreted as a mineral deficiency.
The practical question is not “Which mineral stops sensitive teeth?” It is “Why has the dentine become exposed — and is the wider diet helping or harming the situation?”
Could HTMA add useful information?
Sometimes — when mineral nutrition is genuinely part of the wider picture.
Hair Tissue Mineral Analysis measures selected minerals and elements in a hair sample and can provide additional information about longer-term mineral patterns.
It may become more relevant when tooth sensitivity sits alongside:
- A very restricted or poor diet
- Multiple supplements
- Broader mineral-nutrition concerns
- Digestive or absorption concerns
- Other health issues that make the nutritional picture worth exploring
The findings should be considered alongside:
- Diet
- Health history
- Medicines and supplements
- Symptoms and other concerns
- Appropriate clinical or laboratory information where useful
HTMA may contribute to the wider nutritional assessment. It cannot tell whether a particular tooth has a cavity, crack, exposed dentine or diseased pulp.
When could “sensitivity” be something more?
Classic dentine hypersensitivity is usually short and triggered.
The pain appears with cold, air, touch or sweetness and then settles.
A different pattern deserves attention.
See a dentist if:
- One tooth suddenly becomes much more sensitive than the others
- Pain continues after the cold or hot trigger has gone
- You have spontaneous toothache
- Pain wakes you at night
- It hurts to bite or chew
- The tooth looks cracked or chipped
- You can see a cavity or dark area
- There is swelling, pus or a bad taste from one area
- The pain is steadily getting worse
- Several weeks of sensible home treatment have made little difference
“Sensitive teeth” is a useful description of what you feel. It is not a diagnosis of what is causing it.
What is new right now? — August 2026
The sensitive-tooth aisle is full of competing claims. Newer research is helping separate formulations that genuinely reduce pain from products that mainly rely on marketing.
2025 — what works over the longer term?
A systematic review examining randomised trials with at least six months of follow-up found that several professional desensitising approaches produced meaningful long-term reductions in pain.
The review also found that not every treatment performed equally well.
That reinforces an important message: “desensitising treatment” is not one single intervention.
2026 — newer stannous-fluoride toothpaste trials
Two controlled trials published in 2026 studied an anhydrous 0.454% stannous-fluoride toothpaste in adults with dentine hypersensitivity.
Twice-daily use produced progressive reductions in clinician-measured and patient-reported sensitivity over 28 to 56 days compared with a standard fluoride toothpaste.
That is useful because it supports the everyday message that the active formulation matters — and consistent use over weeks can matter more than expecting instant relief after one brushing.

The bottom line
Sensitive teeth can often be improved — but first work out what you are treating.
If exposed dentine is the problem:
Use an evidence-based desensitising toothpaste. Use it consistently. Reduce acid exposure. Brush gently.
If the sensitivity is linked to gum recession, address the recession too.
And if the pain is persistent, spontaneous, worsening or concentrated in one suspicious tooth:
Do not keep changing toothpaste. Let a dentist find the cause.
